Face Mask Election Form

It is a top priority of Yukon Public Schools to promote health and safety in our learning and working environments so that our students can remain in full-time, in-person learning this school year with layered prevention strategies in place.

Therefore, it is immediately required that all YPS students, employees, and visitors must wear a face covering that covers both the nose and mouth when on YPS property and in YPS vehicles. YPS will provide a mask to ANY student, staff member, or visitor who does not have one.

OPT-OUT OPTIONS

If a parent, legal guardian, or eligible student indicates that compliance with mask use is not possible due to medical, religious, or strong personal reasons, the parent, legal guardian, or eligible student may request an exemption in writing.

To Be Completed by Parent, Guardian, Legal Custodian, Foster Care Provider, Student 18 Years of Age or Older, or Student Otherwise Authorized to Provide Consent.
