With the authorized signature below, the undersigned team certifies that all athletes entered in ANY meet run by Charles River Aquatics during the 2023-2024 SCY season at Boston Sports Institute are registered with United States (USA) Swimming and understands that a fine of $100/athlete may be incurred for each entry not registered with USA Swimming. Additionally, with the authorized signature below, the undersigned team certifies that all coaches on pool deck at the meet are registered with USA Swimming.
On behalf of the undersigned team, I certify that all athletes and coaches participating in any Charles River Aquatics run swim meet ata BSI do so voluntarily, understand the risks inherent in participation, and assume all risks related to participation including, but not limited to; accident, illness, permanent disability, or death, including personal, bodily or mental injury of any nature, and possible exposure to the virus that causes COVID-19.
With the authorized signature below, the undersigned team certifies that ALL ATHLETES in this meet entry do FOREVER RELEASE Charles River Aquatics, Inc. (CRA), United States (USA) Swimming, New England (NE) Swimming, Inc., Boston Sports Institute, their respective trustees, officers, employees, volunteers, students, agents and assigns from any cause of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence now or in the future against, Charles River Aquatics, Inc. (CRA), United States (USA) Swimming, New England (NE) Swimming, Inc., Boston Sports Institute on account of personal injury, bodily injury, property damage, death or accident of any kind, arising out of or in any way related to participation howsoever the injury is caused.
The parties agree that to the extent they sign electronically below, their electronic signature is the legally binding equivalent to their handwritten signature.
