By providing this information to Sebago Sailing, I certify that I am at least 18 years old and the above information is truthful and correct. I certify that I am fully capable and able to operate the above referenced vessel competently and safely and will be fully responsible for the safety of anyone who sails the selected sailboat. I understand that Sebago Sailing will add this to my file / Rental Agreement and this document along with any additional sailing resume document will also become part of my charter contract, as an addendum to that contract. I also authorize that Sebago Sailing to verify that all of the above information is correct.
