Please use this form to enter all requests for model home water heaters. Also, as you may know, not all builders have model home programs with us so please keep that in mind when filling out this form. Our model home submital process goes as follows:

 

1) Distributor issues contractor a no charge model home water heater (If builder has model home program)

2) This form is filled out to completion and no charge invoice attached (option to attach imbedded within form)

3) Request is reviewed

4) If approved, a credit will be issued to the distributor for their purchase price of the heater from us
