This form will take between 10 and 15 minutes to complete. Please make sure you have the details required, including:
- Coach code
- Activity overview (format, title, provider, completed date)
- Documentation of completion where applicable (e.g. certificate of completion)
- Reflection of learning, and how it will impact your coaching
Each submission is independently assessed, which is why it can take up to 4 days to process your submission. For your submission to be accepted, and for CPD credits to be awarded, you need to provide comprehensive responses to give a complete understanding of the learning you have accomplished. These have to be educational experiences that are considered above and beyond your day-to-day activities, and that have taken place in the last 12 months.
