This form should be filled out if you require a refund for tickets purchased in the LTA Youth Schools ballot.
The teacher that opted the school in should be the one that completes the form.
Please remember, that when you complete this form, you will receive a refund for all the tickets in the package.
Once we receive the refund request, we will process this as quickly as possible and have the funds back in the account within 10-15 days.
