This form is for those on their Padel Instructor qualification to log their first aid training.
Your certificate must be for one of the following OFQUAL Level 3 courses:
- Emergency First Aid at Work (EFAW) - 6 hours
- First Aid at Work (FAW) - 18 hours
- Paediatric First Aid (PFA) - 12 hours
Your certificate will be rejected if it is not one of the above courses.
Online courses are not accepted, and training must be delivered face-to-face by a recognised provider in Great Britain.
If your first aid course was run by one of the LTA's Coach Development Centres, your training record will be automatically updated and you do not need to submit your certificate here.
