I/we, the undersigned parent(s)/guardian of the minor child named above, hereby grant permission for my/our child to participate in all activities in and around SAL&M as part of the program described below. In addition, I/we hereby grant permission for my/our child to participate in all field trips which may include travel, walking through wooded areas, climbing the tower, and other activities, including but not limited to those activities included in the program description. Further, I/we agree to assume all risks and liabilities associated with my/our child’s participation in said program(s) and to hold SAL&M harmless from all claims which may arise as a result of participation. In case of emergency, SAL&M has permission to take my child to the nearest hospital.

LUNCH
Friday lunch (pizza) is included with camp fee. Though packed lunches are welcome, lunch can be purchased in advance Monday - Thursday for $6.00 per day. Input the number "1" in the days you would like to purchase lunch. If you are registering for multiple weeks and would like lunch each week for a particular day, put in the total number of weeks. For example, if you register for three weeks and want Tuesday lunch each week, put "3" in the HoneyBaked Ham line.

Check the option that applies to receive the discount. If you are registering more than one camper, apply the discount of the total number of camps for both campers to each campers total. For example, if you are registering a 1st grader and a 3rd grader for one camp each, apply the 5% discount on each registration form. *
Cancelation & Refund Policy SAL&M
We reserve the right to combine or cancel classes that have not met the minumum enrollment.  Campers will be placed in an alternative class when possible.  If SAL&MM cancels a class, payment will be fully refunded.  No refunds will be granted on or after the first day of the camp session.  Contact Summer Camp registrar at 904.829.0745 x223  x208 or x201 for questions about registration or for more information about cancellation and payment options. All registrations subject to camp availability.
