Indoor Facility Request Form

This Form is to be used by individuals or organizations seeking a permit to use an Indoor Facility at a Recreation Center of the San Francisco Recreation and Parks Department

This Form is not for Kezar, Hall of Flowers, Stern Grove, Harvey Milk or the Randall Museum (they have different procedures)
